Telehealth Informed Consent

Introduction

Telehealth involves the delivery of healthcare services using electronic communications, information technology, or other means between a healthcare provider and a patient who are not in the same physical location. By using the telehealth services made available through the HELMD platform, you consent to receiving care through these technologies.

Nature of telehealth services

HELMD is a consumer-facing platform operated by Helmd LLC that connects patients with licensed clinicians affiliated with independent professional medical groups (the "Provider Groups") for the evaluation and potential treatment of erectile dysfunction. Clinical services — including the review of your questionnaire and any prescription decisions — are provided exclusively by clinicians employed or contracted by the Provider Groups. Consultations are typically conducted asynchronously through the secure HELMD platform. Your clinician will review the medical history, current medications, and other health information you submit to determine whether prescription treatment is appropriate for you.

Benefits of telehealth

Telehealth offers improved access to healthcare by enabling you to consult with qualified clinicians without the need for in-person visits. Benefits include convenience, reduced travel time, and the ability to receive care from the privacy of your own location.

Risks and limitations

As with any medical consultation, telehealth has potential risks and limitations, including but not limited to:

Prescriptions and compounded medication

If a clinician affiliated with a Provider Group determines that a prescription is appropriate for you, the prescription may be for a compounded medication prepared by a licensed U.S. compounding pharmacy. Compounded medications are prepared based on a patient-specific prescription and are not approved by the U.S. Food and Drug Administration (FDA). The FDA does not verify the safety or effectiveness of compounded drugs. You acknowledge that compounded medications may carry different risks than commercially manufactured drugs. Your responsibilities

You agree to provide accurate, complete, and current information regarding your medical history, current medications, allergies, and other health-related details. You agree to inform your clinician of any changes in your health status. You understand that withholding or providing inaccurate information may affect the safety and effectiveness of any treatment prescribed.

Privacy and confidentiality

Your health information will be protected in accordance with applicable federal and state privacy laws, including HIPAA. Information may be shared with the dispensing compounding pharmacy to fulfill your prescription, with the Provider Group through which your clinician practices, and with other entities as described in our Privacy Policy or as required by law.

Right to withdraw consent

You may withdraw your consent to telehealth services at any time by contacting us at support@helmd.co. Withdrawal of consent will not affect any services already provided.

Emergencies

Telehealth is not intended for medical emergencies. If you are experiencing a medical emergency, call 911 or go to the nearest emergency room immediately. Do not rely on the HELMD platform for emergency care.
