Why a daily version exists at all
Every drug has a half-life, which is how long your body takes to clear half of a dose. Sildenafil, the active ingredient in Viagra, has a half-life of about 4 hours. Tadalafil has one of about 18 hours.
That difference is why you can take tadalafil daily and not sildenafil. If you took a small dose of sildenafil every morning, it's gone by lunchtime. It never builds up. Tadalafil does build up. Taken at the same time each day, the level in your blood rises over roughly five days and then holds steady.
Contrast that to on-demand treatment: taking a pill before sex creates a concentration curve. The drug climbs, peaks, and falls away, and you need the good part of the evening to land inside that curve. Daily dosing flattens the curve.
Daily dosing does not work on day one. Because it depends on building up, a single tablet is not a real test. Give it a couple of weeks of consistent use before deciding anything.

What it is approved to treat
The FDA has approved tadalafil for several different uses.
Taken as needed for ED: The usual starting dose is 10 mg before sex, adjustable up to 20 mg or down to 5 mg. It has been shown to improve erectile function for up to 36 hours after a dose, which is where the "weekend pill" nickname comes from.
Taken daily for ED: The starting dose is 2.5 mg at roughly the same time each day, regardless of when sex happens. It can be increased to 5 mg based on how well it works for you.
Taken daily for an enlarged prostate: 5 mg once a day.
Taken daily for both: Also 5 mg once a day.
Nothing in the approved labeling says daily dosing produces better erections than taking a pill before sex. Both are approved, and what daily changes is when the drug is in your system, not how well you perform.
What the heart research shows
An erection is, mechanically, a blood flow event. Arteries have to relax and let a lot of blood in quickly. That happens because the lining of your blood vessels releases nitric oxide, a signaling molecule that tells the vessel walls to loosen. Tadalafil works by protecting that signal so it lasts longer.
That vessel lining is not only in the penis. It runs through every artery in your body, including the ones feeding your heart. When it stops working properly, that is the first stage of the process that eventually narrows arteries. It is a whole-body problem, not a local one.
The question researchers have been asking is: if a drug supports that signaling pathway everywhere, do the men taking it end up healthier, generally?
In 2024, Kloner and colleagues published an analysis in Clinical Cardiology looking at more than 29,000 men, comparing those who had taken tadalafil against those who had not. The men taking tadalafil had 44% lower all-cause mortality, 55% lower cardiovascular death, and 19% fewer major adverse cardiac events, meaning heart attack, stroke and heart failure. The two groups were matched for whether they were taking blood pressure, diabetes and cholesterol medication, so this was not simply healthy men against sick ones.
The biology is consistent with this. Drugs in this family are already approved to treat pulmonary arterial hypertension, a serious condition of the blood vessels in the lungs. That is direct proof this class does protective work on blood vessels well outside the pelvis.
What this does and does not prove
This was an observational study. Researchers looked at what happened to men who did and did not take tadalafil. They did not randomly assign it. That matters, because men who get prescribed tadalafil are different from men who never are. They have seen a doctor. They are well enough to be having sex.
Researchers call these differences confounders, and matching for medications handles some of them but not all. Nobody measured how much these men exercised or what they ate. To prove tadalafil protects the heart, you would need a trial that randomly assigns it and tracks cardiac outcomes over years. That trial has not been done, and tadalafil is not approved to prevent heart disease.
What is fair to say: it is a large study, the biology and chemistry behind it is strong, as is the observational evidence. For a man who needs treatment for ED anyway, that’s a green flag.
The prostate benefit
As men age the prostate tends to enlarge, a condition called benign prostatic hyperplasia, or BPH. What men notice is changes in urinary function: going often, getting up at night, sudden urgency, a weak stream.
It is common. The National Institute of Diabetes and Digestive and Kidney Diseases, part of the National Institutes of Health, estimates BPH affects 5 to 6% of men aged 40 to 64, rising to 29 to 33% of men over 65.
Tadalafil at 5 mg daily is separately approved to treat those symptoms, and approved again for men who have both BPH and ED. That means one tablet covering two problems that often show up together.
The mechanism is not fully understood. Researchers believe tadalafil relaxes smooth muscle in the prostate, the bladder and their blood supply, but this has not been formally established.
Who it suits
Daily tadalafil tends to fit men who have sex more than about once a week or unpredictably, who find the timing requirement itself stressful, who also have urinary symptoms from an enlarged prostate, or who simply prefer a routine to a decision.
It also suits men who are being treated for ED and want the cardiovascular evidence working in their favor. If you need treatment anyway, the daily version is the one the long-term research has followed.
It fits less well if sex is infrequent, or if you would rather not take something every day.
Dose adjustments apply in some situations. Reduced kidney function may mean starting at 2.5 mg. Certain medications, including some HIV and antifungal drugs, cap the dose at 2.5 mg.
Safety
The common side effects are headache, flushing, a stuffy nose, indigestion and back pain. Most ease with continued use.
The one absolute rule is nitrates: nitroglycerin in any form, isosorbide, and the recreational nitrites sold as poppers. Combined with tadalafil they can cause a dangerous drop in blood pressure. If you take a nitrate for any reason, this medication is not an option.
Alpha-blockers, significant heart disease, uncontrolled blood pressure, and serious liver or kidney problems all need proper assessment.
Urinary symptoms should be evaluated before being treated. Other conditions cause the same symptoms, and prostate cancer can exist alongside an enlarged prostate.
